YFS POLICIES
TUITION REMINDERS
Youth First Sports will process tuition payments on the first of each month per program agreement. Some agreements are month-to-month all year-round. Others, are per season or a set number of weeks.
A required cancellation email should be sent to: YouthfirstSports@gmail.com 30 days prior to participant's last class in month-to-month all year-round programs. No cancellation notice required for seasonal or programs with set number of weeks.
Rain cancellation/postpone
If rain is in the forecast and classes are cancelled (we will follow school policy) we will work with the school and make every effort to schedule a make up class. There is no guarantee any number of classes per month for the monthly tuition.
Make-ups
There are no make-ups for classes missed due to personal matters. Participants who will miss more than one class sequentially due to illness or injury should contact YouthFirstSports@gmail.com for possible make-up options.
WAIVER/INDEMNIFICATION
In consideration of being allowed to participate in any way in Naybahud Youth First LLC sports program, and related events and activities, the undersigned:
Submission of this form indicates you have read and agree to the terms of our waiver and policies.
As parent/legal guardian of the participant named herein, I hereby represent that the participant has been examined by a pediatrician and is physically fit to participate in Youth First Sports. I understand there are inherent risks in participating in this athletic program.
I hereby accept responsibility for and agree to pay any and all costs of medical treatment resulting from any injury suffered by my child as a result of his/her participation in Youth First Sports.
I further agree to indemnify and hold harmless Youth First Sports, its agents, servants, employees and/or representatives from any and all liability, damage, cost or expense arising out of my child’s participation, of every kind and nature, in Youth First Sports events.
In the event that I cannot be reached in an emergency, I hereby give permission for care to be administered by a qualified Youth First Sports staff member, EMT, physician/staff of a hospital, or any other qualified individual to provide any medical treatment deemed necessary for my child.
Youth First Sports may use photographs and/or videos of my child while participating in Youth First Sports sponsored activities.
Agree that the parent(s) and/or legal guardian(s) will instruct the minor participant that prior to participating he or she should inspect the facilities and equipment to be used, and if the participant believes anything to be unsafe, he or she should immediately advise his or her coach or supervisor of such condition(s) and refuse to participate.
Acknowledge and fully understand that each participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inaction, or negligence, but the action, inaction and negligence of others, or the condition of the premises or of any equipment used. Further, that there may be other risks not known to us or not reasonably foreseeable at this time.
Release, waive, discharge and covenant not to sue Naybahud Youth First LLC and Sports Programs its affiliated clubs, their respective administrators, directors, agents, and other employees or the organization, other members/participants, sponsoring agencies, sponsors, advertisers, and if applicable owners and leasers of premises used to conduct the event all of which are hereinafter referred to as "releases," from any and all liability to each of the undersigned, his or her heirs and next of kin for any and all claims, demands, losses or damages on account of injury, including death and damage of property, caused or alleged to be caused in whole or in part by the negligence of the releases or otherwise.
